Requests must include ALL of the following before they are considered “complete” and ready for review:
- Cover letter indicating requested amount and the purpose of the grant
- Organizational overview
- Program description, including demonstration of need, audience served and measurable outcomes (both expected and already achieved)
- Organization’s budget
- Program budget
- Board of directors
- 501©3 documentation
- Statement of nondiscrimination – Please copy the following statement on your organization’s letterhead, sign and date. This statement is required for a funding request to be considered:
